Please note that the forms below are for your information only.  A properly filled out copy of each form will be either mailed or e-mailed to you for you to sign and return to us with your deposit or given to you to sign when you pick up your puppy.

                                                              Sales Agreement

Be it known,            (our names)                       of            (our kennel name)          do agree this day  ___________________ (current date)  to sell to   ____________________________  (buyer)  the following puppy or dog (hereafter referred to as puppy)  __________________________________ for the purchase price of  ____________  plus shipping cost of _________, if shipping is applicable, for a total amount due of ______________.   If the seller will be required to hold the puppy described above for more than  five (5)  days after the date listed above, a deposit of __________  will be required in order to hold the puppy.  This deposit is non-refundable unless the seller is unable to supply the chosen puppy or a suitable substitute.  If  buyer does not take possession of the puppy within   30    days after the puppy is weaned (usually  8 weeks of age) or if already weaned at the time of sale, within   30   days  of the date of sale listed above,  the buyer will be considered to have changed his/her mind about purchasing said puppy and the deposit will be forfeited unless separate agreement has been made in writing to extend the holding period. 

Full payment of the total price listed above must be made at the time of the buyer taking possession of said puppy or no less than  twelve (12)    days prior to the scheduled shipping date of  ____________  if the puppy is to be transported by a third party.   Seller will not relinquish possession of the puppy identified above until full payment has been received.  

Buyer agrees that he/she will not resell this puppy to a broker, pet store or research facility.  Buyer also agrees not to place this puppy in an animal shelter or similar facility at any time.  If at any time after taking possession of the puppy described above, the buyer should decide that he/she no longer wants said puppy; the seller  ________________________   will be notified and given the opportunity to buy back the puppy at a price not to exceed the original purchase price listed above (not including shipping cost) or to help locate a suitable new home for the puppy.

        Health Guarantee and Return Policies

!/we           (our  names)                 of          (our kennel name )     certify  that the puppy or dog described as follows  _________________________________ (hereafter referred to simply as puppy)  was to the best of our knowledge healthy and free from disease on  ______________  (date), at which time the puppy was relinquished from our possession into the care and possession of the buyer  _______________________________  or if the puppy is to be shipped, the above date is the date when the puppy received or is scheduled to receive its check up and health certificate from our veterinarian and we certify that said puppy was healthy and free from disease when it was relinquished from our possession into the care and possession of the transporter.  We also certify that this puppy was immunized and wormed as stated on the accompanying health record.  If the buyer's  veterinarian performs vaccinations, wormings or other procedures on this puppy that result in sickness or injury, the seller will not be held responsible for any resulting health problems.

It is recommended that the buyer have the puppy described above examined by a licensed veterinarian within  ______   hours of taking possession of the puppy.  If at that time it is determined that the puppy is sick or had contracted a disease prior to the sale of said puppy, the puppy will either be replaced or a portion of the purchase price refunded.  Replacement or refund will be at the sellers discretion and will depend on the availability of a suitable replacement, the feasibility of returning the original puppy and whether or not the buyer wishes to keep the original puppy.

If at any time up to ______  months of age the puppy identified above is determined to have a genetic defect, the buyer may be given a replacement puppy or a refund, at sellers discretion.  Any amount to be refunded on a defective puppy will be determined by the seller based on the current value of a pet quality puppy of the same breed.

Any amount refunded to the buyer for whatever reason shall not exceed the original price paid for the pupppy as listed on the sales agreement, less the deposit and any expenses incurred by the seller for shipping or other method of retrieving the puppy from the buyer.  Any refund or replacement will require written verification by a licensed veterinarian documenting the defect or health problem and the cost of treatment if any.  Any refunds for genetic defects will require written verification that the defective puppy has been spayed or neutered.

This guarantee does not cover any injury that may occur to the puppy after it has left the possession of the seller.

I/we, the buyer, do agree to have the puppy identified above examined by a licensed vererinarian within  _______ days or taking possession of said puppy and understand that failure to do so shall void the above portions of this document pertaining to sickness and disease and shall relieve the seller of any legal responsibility for the health of the puppy identified above.  Furthermore,  I/we have on this day ________________ read and understood the terms and conditions set forth above and do agree to the terms as stated.
